Serve ‣ Youth Leader | Application

This application is to be completed by all those seeking to volunteer in a Youth Ministry position at Central Communication Church.

This is not an employment application form. It is an application to assist the church staff in establishing ministry teams and it is a screening form used to help Central Youth Ministry provide a safe and secure environment for the students who participate in our programs and use our facilities. Thank you for your understanding of the necessity of such a form.
